INTRODUCTION HAZARD IDENTIFICATION (HAZID)

Hydrocarbon processing industry is categorized as a high risk industry due to the hazards containing either in the material processed or the operations used in its facilities. Because of its vulnerability, a simple event, either man-made or natural one may lead to significant disasters. Flixborough, Bhopal and Piper Alpha disasters are three classic examples of major accident causing serious loss of life and/or property. A deep understanding on safety-related aspects of the materials and processes or operations implemented in hydrocarbon facilities is therefore critical to operate this kind of industry in safe manner and preventing it to go wrong. Hazard identification ‘the identification of undesired events that lead to materialization of a hazard, the analysis of the mechanisms by which these undesired events could occur and usually estimation of the extent, magnitude and likelihood of any harmful effects’ This short course is intended to give understanding on HSE problems and several methods of Hazard Identification (HAZID).

INSTRUCTOR

Dr.Eng.Ir. Azis Trianto,M.Sc. dilahirkan di Pekalongan 1 Februari 1969, Posisi akademik beliau sekarang sebagai Assistant Professor pada departemen Teknik Kimia ITB. Beliau menamatkan Sarjana di Departemen Teknik Kimia ITB, Masternya dari Departemen Kimia Tokyo Institute of Technology Jepang, Doktor diambilnya dari deapartemen Teknik Kimia Tokyo University of Agriculture and Technology Jepang, dan beliau juga sempat mengambil Post Doctoralnya di Japan society for promotion of science fellow, Departemen Teknik Kimia Tokyo University of Agriculture and Technology Jepang. Dari tahun 2008 sampai sekarang beliau adalah ketua Graduate Program pada Departemen Teknik Kimia ITB. Di kampusnya beliau mengasuh mata kuliah termodinamika, material & energy balance, chemical reaction engineering, utility system, separation process, chemical process safety & chemical process design. Beberapa penelitian beliau, diantaranya hydrogen recovery from off-gas, propylene synthesis by propane dehydrogenation, synthesis of higher hydrocarbons from methane, gas separation using membrane, and L-Lactic Acid separation. Pada tahun 1997 – 2002 beliau sebagai Research Associate pada Center for Research Energy ITB. Pada tahun 1993 – 1994 beliau juga sebagai Research Associate pada Development Technology Center ITB. Beliau telah mengeluarkan banyak papers, mengikuti conferences dan mempunyai beberapa paten diantaranya “Catalytic reactor with porous membrane partition wall, manufacture of chemical substances using same, and reaction apparatus using same,”  dan “Metode dan Alat Pengukur Permeabilitas Gas melalui Membran Berpori”. Beliau juga melakukan beberapa tugas, diantaranya “Development of Prototype of a Pilot Two-Stage Domestic Waste Treatment Unit”, “Monitoring on the Release of Hazardous Metal Ions from Battery and Electroplating Industries to Aquifer,”, “Study on CO2 Emission in Industrial Sector in Indonesia,”, “Update on General Specification of PT Badak”. Dr.Eng.Ir. Azis Trianto,M.Sc. sudah sering mengajar training ke banyak perusahaan di Indonesia, untuk topik-topik di bidang kompetensinya seperti QRA and RBI Methodology, Quantitative Risk Assessment, Natural Gas Processing and Process Control, Natural Gas Processing, Occupational Safety and Health, Piping Simulation, Laboratory Management, Gas Processing and equipment, dan lain-lain.

INTRODUCTION HAZARDS IDENTIFICATION, RISK ASSESSMENT AND DETERMINING:

Hazard identification (HAZID) and risk assessment involves a critical sequence of information gathering and the application of a decision-making process. These assist in discovering what could possibly cause a major accident (hazard identification), how likely it is that a major accident would occur and the potential consequences (risk assessment) and what options there are for preventing and mitigating a major accident (control measures). These activities should also assist in improving operations and productivity and reduce the occurrence of incidents and near misses.

There are many different techniques for carrying out hazard identification and risk assessment at an Major Hazards Facility (MHF). The techniques vary in complexity and should match the circumstances of the MHF. Collaboration between management and staff is fundamental to achieving effective and efficient hazard identification, risk assessment processesandDetermining control.

BambangPamudji, B.Sc.,SE.has much real experiences, he is former Senior Health and Safety staff at PT. PupukSriwidjaja,PUSRI(Urea Fertilizer Manufacturer) Palembang. He was born in PasuruanEast Java, 14 March 1950. He got hisB.Sc in Public Health from APK Surabaya in 1971 and was graduated from Economic Management Sriwidjaja University, UNSRI, Palembang in 1996. He has certificates in Health, Safety, and Environment (HSE) from Depnakertrans RI/Institutions and Universities in Indonesia, institutions and Universities in UK, and National Safety Council. Experienced in setting up Health and Safety Management System documentation and its application. Experienced internal lead auditor SMK3 in PT. PupukSriwidjaja, and External lead auditor SMK3 for Indonesian Association Fertilizer Producer. He has been teaching industrial training classes for many classes related to topics of his competencies, HSE. Many participants from many companies in Indonesia have participated in his training classes.
